Client Agreement
As the client you agree that it is your choice to receive massage therapy/bodywork, or to partake in yoga/meditation. You acknowledge that you are aware of the benefits and risks of massage/yoga and give full consent for massage or physical touch (reiki) during yoga/meditation. As the client, you understand that there is no implied or stated guarantee of success or effectiveness of individual techniques or series of appointments. You acknowledge that massage therapy, bodywork, yoga & meditation are not substitutions for medical care, medical examination or diagnosis & you have stated all medical conditions that you're aware of and will inform your practitioner of any changes in health status. The health waiver below MUST be filled out before any sessions, classes or hands-on workshops.
Payments
As the client, you are responsible for all charges of any services you've booked at Fisher Wellness. We do not offer refunds unless your session/class/event was cancelled by Fisher Wellness & we do not offer refunds on deposits unless your session was cancelled Fisher Wellness.
