Client Agreement & Consent for Massage and Bodywork Services

This agreement is made between Fisher Wellness, LLC (“Provider”) and the undersigned client (“Client”) for massage therapy and/or bodywork services. By checking the box on the health waiver form / signing the health waiver form, the Client acknowledges and agrees to the following terms:​
1. Scope of Services

Services may include massage therapy, bodywork, cupping, polarity therapy, and other wellness modalities offered by Fisher Wellness, LLC.

All services are provided by licensed massage therapists or other qualified practitioners in accordance with Massachusetts law and professional standards.​

2. Client Health Information & Disclosure
-
The Client agrees to provide complete and accurate health information before receiving services.

The Client will update Fisher Wellness, LLC of any changes to health status, medical conditions, injuries, medications, or other relevant information.

The Client understands that withholding health information may pose a risk and releases Fisher Wellness, LLC from liability if such information is not disclosed.​

3. Nature of Massage & Bodywork

Massage therapy and bodywork are intended to promote relaxation, pain relief, and general wellness.

These services are not a substitute for medical care and are not intended to diagnose, treat, or cure any medical conditions.

Therapists do not prescribe medications, perform chiropractic adjustments, or provide medical advice.​
​
4. Consent to Treatment

The Client consents to receive massage and bodywork services and understands that techniques may involve physical touch.

The Client has the right to ask questions, request adjustments, or stop the session at any time.

The Client agrees to communicate immediately if they experience discomfort, pain, or wish to modify or end the treatment.
5. Professional Boundaries

All services are strictly professional and therapeutic.

Any inappropriate, sexual, or harassing behavior will result in immediate termination of the session, full payment due, and possible permanent dismissal from Fisher Wellness, LLC.​
6. Cancellation & No-Show Policy

At least 24 hours’ notice is required to cancel or reschedule an appointment.

Cancellations with less than 24 hours’ notice, or missed appointments, may result in a charge equal to the full session fee.​
7. Payment

Deposit is due upon booking & remaining balance is at the time of service unless otherwise arranged.

Accepted forms of payment include cash, credit/debit cards, or other approved methods.

As the client, you are responsible for all charges of any services you've booked at Fisher Wellness. We do not offer refunds unless your session/class/event was cancelled by Fisher Wellness & we do not offer refunds on deposits unless your session was cancelled Fisher Wellness. If you do not cancel within 24hrs of your session you will be charged in full.

8. Confidentiality

All client records and health information are confidential and will not be released without written consent, except as required by law.

9. Liability Release
​

The Client acknowledges that massage and bodywork carry some risk, including but not limited to soreness, bruising, or aggravation of existing conditions.

The Client voluntarily assumes full responsibility for any risks or outcomes associated with receiving services.

The Client releases Fisher Wellness, LLC and its practitioners from any claims, damages, or liabilities arising from services received, except in cases of gross negligence or misconduct.​

10. Agreement
By checking the box on the bottom of the health waiver form/signing the health waiver form, I acknowledge that I have read, understand, and agree to the above terms. I consent to receive massage and/or bodywork services at Fisher Wellness, LLC.
